Freedom of Information Act

A request for information must be submitted in writing to the Freedom of Information Act (FOIA) Officer via personal delivery, USPS mail, facsimile, or electronic mail. District 75 will not honor verbal requests for information.

The first fifty (50) pages of black/white, letter-size or legal-size copies are provided free-of-charge. Additional pages thereafter will be charged at a rate of .15 cents per page. Color copies will be charged at the actual cost of reproducing the records. The cost for certifying a record will be $1.00. If documents are to be sent via USPS mail, the District will charge for postage. For electronic copies, the District will charge the actual cost of the recording medium (i.e. CD disc, tape, etc.)
Immunization Report & Other Health Records
5013 requires school districts to make publicly available, by December 1, of each year, the student immunization data that the district is required to submit to the Illinois School Board of Education each year.
